What is Senior Health And Outreach and How Can it Help?
Senior Health and Outreach nurses receive referrals to see seniors from a variety of sources; self, family members, neighbors, physicians and other service providers. The program is designed to support seniors so they can live independently as long as possible and avoid unwanted or premature placement in health care facilities. Nurses work with clients and other care providers to develop a plan of care. Follow-up visits are provided every six months or as necessary to respond to changing needs of clients.
What Can Senior Health And Outreach Nurses Do?
Provide in home assessments to review a client’s medical, social and environmental needs.
Provide health education consistent with client’s needs.
Discuss referrals that are appropriate for the clients and help link them to other community resources and services.
Review medications and set up programs to help ensure their safe administration.
Provide follow-up visits and telephone calls as needed.
Blood pressure and health screening at various locations in the community.
Who are Senior Health And Outreach Clients?
Seniors (60 years and older) who live in Nevada County. Everyone in this age group is eligible for services regardless of income.
