The Confidential Morbidity Report forms are meant for use by healthcare providers in Nevada County to report communicable diseases to the health department. The Disease Reporting flyer lists selected communicable diseases by their reporting urgency. The Community Health Department appreciates and values local healthcare provider participation in communicable disease control.
