Nevada County Online Service Request Form.
The form requires the Microsoft Internet Explorer browser.  In addition "All Pop-Up Blockers must be turned off."


If you just have a question, use our simple CONTACT US form instead.

On the Form you can select a "Category" which will bring up a selection of "Service Names."
For example, you will find "Insects and disease" associated with the "Agriculture" Category (where you might input a mosquito problems), or "Pot hole" associated with the "County Roads" Category to get a pot hole filled.

Please select the Category and Service Name that best fits you request. If you don't find it at all, please use the simple CONTACT US form instead. Thank You.

The online form will open in a new browser window. Refer to the instructions below to assist you. You may get a window asking you to accept our "security certificate." Answer YES.
NOTE: You must use the Microsoft Internet Explorer browser to use our online service request system.
  1. The screen should open with a blinking cursor in the FIRST NAME box. Type your first name.
  2. Hit the TAB key to advance to the LAST NAME box, and type that in. Hit TAB again.
  3. In the STREET NUMBER box, type just the NUMBER part of your address. When finished, wait a few moments for a new SELECT STREET ADDRESS box to appear to the right.
  4. Click the down-arrow on that box. You should see the name of your street there; if so, click on it to select it. (If not, select the "Not found in list" option.)
  5. If applicable, click in the ORGANIZATION box and type your business or organization.
  6. In the CONTACT YOU HOW? area, click the best way to contact you.
  7. Depending on your contact method, a box may open in which you can type information (your Email address or day/evening phone numbers). If so, please type that in.
  8. Click the down-arrow next to the CATEGORY box. Select the category of your request from the list by clicking on it.
  9. Click the down-arrow next to the SERVICE box. Select the type of service you are requesting.
  10. Depending on the Category and Service, other boxes may appear below. Please click in each one and fill in the requested information.
When finished, click the SUBMIT button at the bottom. (If you have missed any required fields, a box will pop up to inform you; close that box and fill in the required field.)

The Customer Receipt screen now appears. Write down the number that appears in the REQUEST ID box and keep for your records.
